ODP Coach/Chaperone Background Checks

  1. Click Here to Register
  2. Click on Registration 
  3. Select Coaching Registration and continue.
    • Use Option 1 if you have a username a password.
    • Use Option 2 if you have forgotten your username and password.
    • Use Option 3 if you don’t have a username and password.
  4. Complete your profile and click on Create New Account.
  5. Family Member List
  • ​Make sure only your name has a check mark in the middle box that says Coach/Admin.
  • In the relationship box, select no relationship.
  • Enter your date of birth.
  • Select your gender and continue.
  • On the Family Member Review List page, make sure you have your name and date of birth recorded accurately and then continue.
  1. Coach/Administrator Application
  • In the second box, select Coach Registration from the drop down menu.
  • Click on Create Application.
  1. Personal Information
  • Scroll down to ID Information section.
    • Select Drivers License and enter your license number and expiration date respectively. 
  1. Level of Play
  • Scroll down to the Select Level Play section and put a check mark in the ODP box.  Make sure you put check mark in at least one box.
  1. Role
  • Put a checkmark next to your role: Assistant Coach, Team Captain, Head Coach or Team Assistant
