National Referees
All National upgrade and maintenance assessments are $200 plus mileage. Payment for the assessment only is embedded in the Referee Assessment Request Form. After submitting the form, you will be taken to a Google Form via STRIPE payment. Do not begin to submit the Assessment Request form without having your STRIPE login or a credit card present.
A request for payment for mileage will be sent to you after a Referee Coach is assigned.

Regional Referees & Candidates
All Regional Referee upgrade and maintenance assessments are $100. Payment is embedded in the Referee Assessment Request Form. After submitting the form, you will be taken to a PayPal payment form. Do not begin to submit the Assessment Request form without having your PayPal login or a credit card present.
Referee Academy Members & Candidates
Referee Academy Candidates may request their first assessment for free. Please notate this in the comments of the Request Form, request a Developmental Assessment, and ignore the automated PayPal payment request.
Current Referee Academy Members must pay the the assessment fee for their annual requirements to stay in the Academy.
Requesting an Assessment
Use the Assessment Request Form in order to request an assessment. The longer lead time you provide, the higher the likelihood that an assessor can be scheduled for your match. Do not wait for ARs to be assigned to enter your request.
Please note that each game requires an individual request to be submitted.
Notes
You will automatically be sent an email confirming your submission request which contains a link to edit your request. If any game details change (date, time, location, etc.), you must edit your submission, and also email the SDA with the updated changes.
If there are any games changes or cancellations with less than 48 hours notice, notify both the SRA and the SDA via email, so the assigned assessor may be notified.
Out of state assessments: Email both SRA and the SDA prior to requesting out of state assessments.
